Diolch am eich cais am hyfforddiant gan eich cyfoedion o’r Gwasanaeth Cymorth Proffesiynol i Waith Cymdeithasol (GCPGW). Er mwyn dechrau’r broses i’ch cofrestr, llenwch y ffurflen hon os gwelwch yn dda. Bydd hyn yn rhoi mynediad i chi at blatfform diogel GCP.

Ar ôl i chi gyflwyno’r ffurflen hon, byddwch yn derbyn ebost ar wahân gan GCPGW yn eich gwahodd i ymuno a Phlatfform GCPGW. Bydd yr ebost hwn yn cynnwys cyfarwyddiadau, gam wrth gam, ar sut i gofrestru ar y gwasanaeth diogel, sut i ddod o hyd i hyfforddwr ac i wneud apwyntiad. Y rheswm dros y broses dau gam yma yw sicrhau eich diogelwch a’ch preifatrwydd.

Nodwch bydd unrhyw wybodaeth a roddir gennych ar y ffurflen hon yn galluogi ni i roi gwell cefnogaeth i chi a chaiff ei ddefnyddio i’r amcanion i’ch cofrestru chi ar blatfform Gwasanaeth Cymorth Proffesiynol yn unig.

Request for Peer Coaching

Thank you for requesting peer coaching from the Social Work Professional Support Service (SWPSS). Please complete this form to commence the registration process which will give you access to the secure PSS platform.

Once you have submitted this form, you will receive a separate email invitation from the SWPSS to join the SWPSS Platform. This email will include step-by-step instructions on how to register on the secure service, find a coach and book an appointment. The reason this is a two-step process is to ensure your security and privacy.

Please note that any information you provide on this form will enable us to better support you and will be used solely for the purposes of registering you on the Social Work Professional Support Service platform.

Emails and listing information processed through the Social Work Professional Support Service are stored within the BASW UK email service, the Strengthening Practice email system, and a web-based platform hosted by Microsoft and are subject to BASW’s Data Protection and privacy policies. Your data will be used only for participation in the Professional Support Service and will not be used for any other purpose without your express knowledge and consent.
