Diolch i chi am gynnig bod yn hyfforddwr gwirfoddol gyda Gwasanaeth Cymorth Proffesiynol i Waith Cymdeithasol (GCPWC).

Wedi i chi gwblhau’r ffurflen ar-lein isod, fe ystyrier eich bod yn deall mai rôl wirfoddol yw hon ac y byddwch yn cynnig hyd at bedair awr i hyfforddi pob mis am gyfnod o chwe mis.

Unwaith y  byddwch wedi cyflwyno’r ffurflen, bydd aelod o dîm gweinyddol GCPWC yn cysylltu â chi i ofyn am dystiolaeth adnabod i ddangos pwy ydych fel y gallem eich paratoi ar gyfer Platfform GCPWC. Y rheswm am y broses gwirio aml gam hwn yw i amddiffyn eich diogelwch a’ch preifatrwydd chi a’r rhai sy’n cymryd rhan.

Unwaith bydd eich tystlythyrau wed cael eu cadarnhau gan aelod o’r tîm gweinyddol, byddwch yn derbyn ebost ar wahân oddi wrth GCPWC, yn eich gwahodd i ymuno a phlatfform GCPWC. Bydd yr ebost yn cynnwys cyfarwyddiadau, gam wrth gam, ar sut y gallwch gofrestru ar y gwasanaeth diogel, sut i  gyflwyno eich manylion bywgraffiadol a sut i reoli eich calendr a’ch apwyntiadau.

Nodwch, os gwelwch yn dda, bydd y wybodaeth a roddir gennych ar y ffurflen hon yn cael ei ddefnyddio i amcanion i’ch cofrestru chi fel hyfforddwr gwirfoddol ar blatfform GCPWC  yn unig.

Registration Form for Volunteer Coaches

Thank you for offering to be a volunteer coach with the Social Work Professional Support Service (SWPSS).

When you complete the online form below, you are deemed to understand that this is a volunteer role and that you will need to offer up to four hours of coaching a month for a period of 6 months.

Once you have submitted the form a member of the SWPSS administration team will contact you to request evidence of your identity and credentials so that you can be set up on the SWPSS Platform. The reason this is a multiple-step verification process is to protect the security and privacy of yourself and the participants.

Once your credentials have been confirmed by a member of the administration team, you will be sent a separate email invitation from SWPSS to join the SWPSS Platform. This email will include step-by-step instructions on how to register on the secure service, enter your biographical details, and manage your calendar and appointments.

Please note that the information you provide on this form will be used solely for the purposes of registering you as a volunteer coach on the SWPSS Platform.

Emails and listing information processed through the Social Work Professional Support Service are stored within the BASW UK email service, the Strengthening Practice email system, and a web-based platform hosted by Microsoft and are subject to BASW’s Data Protection and privacy policies. Your data will be used only for participation in the Professional Support Service and will not be used for any other purpose without your express knowledge and consent.
